Assalamu Alaikum dear brother,

Before we add you to the core group, where we will begin the sessions of the Fitrah–Burhan Model, I request you to first fill out this questionnaire.

The intention is not to judge your writing skills, knowledge, or ability to express yourself. We simply want to understand whether you are genuinely willing to be part of this journey and ready to give it the time and commitment it requires.

This project is being prepared with resources, time, and effort. I hope you will value that by being sincere and committed throughout the journey.

InshaAllah, I will do my best to support you throughout this process. From your side, I ask for only two things:

1) Honesty     2) Commitment

Please answer from your heart. There is no “right” or “wrong” answer. Your answers will help us understand you better and see whether this journey is suitable for you at this stage.

Don’t rush through it. Take your time, reflect, and then write your answers.

14. Commitment

I understand that this is a commitment-based journey. I agree to be honest in the information I provide and to make a genuine effort to follow the commitments of the programme.

I also understand that if the information provided is intentionally false, or if I repeatedly fail to honour the commitments of the programme, my participation may be discontinued.
